1. I confirm that this test is for peace-of-mind use only.
2. I am at least 18 years old.
3. I have the authority to order a DNA test.
4. I understand that there is a greater risk of contamination, insufficient DNA or DNA breakdown if sample is collected by untrained persons.
5. I understand that there is no refund if the test fails due to contamination, insufficient DNA or DNA breakdown, and there will be additional expense if the test is repeated with a new sample.
6. I agree to the following:
a. Test provider will perform the requested test using the sample submitted.
b. Test provider is not responsible for incorrectly collected samples.
c. Test provider can not verify names of the tested parties, or the source of the sample submitted for testing.
d. Test provider is not responsible for the result obtained and can not be held liable for the outcome of the result and how the report will be used.
